Heartland Health Centers – Touhy
Heartland Health Centers is a federally qualified health center serving Chicago’s north side and nearby suburbs. The clinic network is open to the entire community, including the uninsured as well as those who have Medicaid, Medicare, or private insurance. In addition to the community centers, they also offer school-based health centers and integrated care sites located within agencies providing mental health and substance abuse services.
The Touhy community health center is located on West Touhy Avenue in Chicago. The Touhy center provides primary health care to children and adults, as well as women’s health services.
