Community Health Partnership

Community Health Partnership of Illinois delivers quality, culturally responsive healthcare to their patients and the communities they serve. The partnership is a Federally Qualified Health Center (FQHC) that delivers primary care services for the uninsured and underserved.

The Aurora location of the Community Health Partnership provides primary care services for all ages, including well-child care, cancer and diabetes screening, chronic disease management, and women’s health services. The clinic accepts most third-party insurance, with a sliding fee scale program for those without health care plans. The Aurora Community Health Partnership location offers medical services in Suite D and dental services in Suite C.

The clinic is open Monday and Wednesday from 9AM-7PM, Tuesday and Thursday from 8AM-5PM, Friday from 8AM-3PM, and one Saturday per month from 8:30AM-1:30PM.
