Heartland Health Centers – Lakeview
Heartland Health Centers is a federally qualified health center serving Chicago’s north side and nearby suburbs. The clinic network is open to the entire community, including the uninsured as well as those who have Medicaid, Medicare, or private insurance. In addition to the community centers, they also offer school-based health centers and integrated care sites located within agencies providing mental health and substance abuse services.
The Lakeview community health center provides comprehensive health care to children and adults, as well as integrative medicine, behavioral health and psychiatry services.
